The table below lists one scrapbook, containing samples of forms, circulars and envelopes used in all aspects of the reporting and registering of births, marriages and death in Ontario, in various Canadian provinces and in the United States of America. While the vast majority of the items are blank, some of the Ontario samples are completed and seem to be genuine. The completed documents along with the names of the parties involved are listed below.
Some printed copies of legislation concerning vital statistics and hand-written drafts of forms can also be found in this scrapbook.
